Registration
The following fees apply for the 2012 International Waveform Diversity & Design conference. The registration fee includes a CD copy of the proceedings, Welcome Social (Sun), Lunch (Mon, Tues, Thurs), Thurs Luau Banquet, and Breaks (Mon, Tues, Wed, Thurs). Additional Banquet Tickets may be purchased when registering. Waimea Canyon trip tour, Catamaran Sunset Dinner cruise and extra luau banquet tickets can all be purchased through the registration system, either at the same time you register, or separately by selecting “Tutorial Only” at the registration screen to go directly to the "additional items" section.

Visa Assistance Letters
Speakers, Committee members and paid attendees requiring visa assistance letters must send an email to Patty Woodard: patty@stbeventplanning.com

CANCELLATION POLICY: All refund/cancellation requests must be in writing and received by 11/30/2011.  There will be an administrative fee of $75 deducted from each refund. No refunds will be issued after 30 November 2011. Submit all cancellation/refund requests to wdd12reg@ieee.org.

OFFLINE REGISTRATION: Authors and exhibitiors who do not wish to register online, may fill out the registration form and mail or fax it to IEEE/MCM using the contact information listed below. Mail your check together with the registration form.  Please DO NOT submit the form without payment. Registration forms submitted without payment information will not be processed.
